Community Benefit Network (“CBN,” “we,” “us,” or “our”) operates the Community Benefit Portal, which facilitates enrollment, scheduling, completion tracking, and certification for court-mandated and community benefit programs such as the Hospital and Morgue (H.A.M.) Program.
We respect your privacy and are committed to protecting your personal information. This Privacy Policy explains how we collect, use, share, and protect your information, including information transmitted through SMS, email, and our online portal.
1. Information We Collect
We collect information directly from participants, referral agencies, and partner institutions as part of program operations. This includes:
A. Personal Information
Name
Date of birth
Email address
Mobile number
Mailing address (if provided)
Court case number, referral agency, and offense type
B. Program and Educational Data
Enrollment and payment details
Appointment reservations and attendance records
Course completion, evaluation, and certificate data (Proof of Enrollment and Proof of Completion)
TeleHealth and class participation records via Thinkific or other integrated platforms
C. Technical and Usage Data
IP address, browser type, and device information
Log data from portal usage (for security and audit trails)
2. How We Use Your Information
We use your information for legitimate operational and legal purposes, including:
Processing enrollments and appointment reservations
Sending confirmations, reminders, and progress updates
Verifying completion for court and referral agencies
Issuing Proof of Enrollment (POE) and Proof of Completion (POC) certificates
Generating reports for courts, probation, and referral sources
Maintaining system security and compliance
Providing support and responding to inquiries
3. SMS and Email Communications
By providing your mobile number and/or email address, you consent to receive automated communications related to your participation in CBN programs.
These may include:
Appointment confirmations and reminders
Enrollment and payment confirmations
TeleHealth completion notices
Reschedule prompts
Certificate access notifications
Weekly progress reports
All SMS communications are transactional and not promotional. Message frequency varies depending on your activity and program progress.

You may opt out of SMS messages at any time by replying STOP, or opt out of emails by using the unsubscribe link provided.
However, opting out may prevent you from receiving important updates required for court compliance.
4. How We Share Your Information
Your information may be shared only with authorized entities that play a role in your program workflow:
Referral Agencies / Volunteer Centers (CRA): for intake, progress updates, and completion verification.
Courts and Probation Departments: for official proof of enrollment or completion.
Service Providers: including Thinkific (TeleHealth platform), Twilio (SMS gateway), SendGrid or SES (email delivery), and web hosting/security partners (Hostinger, Cloudflare, WP Cerber).
Medical or Educational Partners: such as hospital and trauma center hosts for in-person visits.
We do not sell or rent your personal information.
5. Data Retention
We retain participant records only as long as necessary to comply with court reporting and verification requirements.
Certificates, attendance records, and audit logs may be retained for up to five (5) years for compliance verification.
6. Data Security
We implement administrative, technical, and physical safeguards to protect your information:
SSL encryption (via Cloudflare)
Role-based access control
Two-factor authentication for administrators
Daily backups
Firewall and brute-force protection (via WP Cerber)
Only authorized personnel have access to sensitive participant data.
7. Your Rights
Depending on your jurisdiction, you may have the right to:

Request deletion of your information, where applicable

Our programs are intended for court-referred participants aged 16 and older. We do not knowingly collect information from children under 13.
